This adult patient's primary concern with their teeth was the crowding of their front teeth.

As a general dentist on the Gold Coast providing Invisalign treatment I was able to offer Invisalign treatment in our clinic to correct the dental crowding.

We discussed the strict requirements of clear aligner treament needing 22 hours day and night wear, removal while eating, cleaning teeth prior to reinserting aligners, need for attachments, interproximal reduction and need for life long retention or risk relapse of crowded dentition. I also gave the option of referal to an orthodontist specialist for treatment who can also provide the option of fixed braces. The patient elected to proceed with Invisalign treatment at our Gold Coast clinic.

The initial ClinCheck animation from Invisalign predicted that we would be able to resolve all crowding with 26 aligners, attachments (marked in red) and interproximal reduction between some of the lower right teeth. The blue dot in the animation highlight movements that are moderately difficult and would require careful monitoring during treatment.

The patient was highly compliant with wearing aligners day and night and changed to new aligners weekly.

As is often the case fine tuning of movements was required and two rounds of additional aligners were utilised to ensure complete resolution of crowding. The first round of additional aligners required 8 aligners and more interproximal reduction to improve position of the upper left lateral and lower left canine. The third and final round of additional aligners consisted of 10 aligners to make finishing improvements of the position of the lower left canine tooth.

The total number of aligners required for this particular patient case was 44 and treatment time with pauses in active treatment to order additional aligners came to just over a year.

At the end of active straightening treatment we then entered the retention phase, maintaining the straight teeth. Teeth are prone to crowding with age and straigthened teeth can what we call relapse, become crowded again. All patients are required to wear removable retainers that look very similar to aligners day and night for three months after Invisalign treatment and then every night forever, or for as long as you want to keep straight teeth. Retainers are made from a thin plastic, discolour and break with time and wear and so are needed to be replaced in future. This patient also elected the highest retention protocol which is to also have fixed wire retainers on the inside surfaces of top and bottom front teeth.
